Job Description
About the Role
We are seeking an organised, proactive, and compassionate Support Coordinator to join our growing team. The ideal candidate will have strong communication skills, a solid understanding of the NDIS, and the ability to coordinate supports that empower participants to build capacity and independence.
Key Responsibilities
- Assist participants to understand their NDIS plan, funding categories, and support options
- Coordinate services, appointments, and referrals to allied health, community supports, and mainstream services
- Build participants’ capacity to manage their own supports and strengthen informal networks
- Monitor plan utilisation and ensure supports align with NDIS goals
- Prepare progress reports, risk assessments, and plan review documentation
- Maintain accurate, audit‑ready case notes and records in line with NDIS Practice Standards
- Respond to participant needs with professionalism, empathy, and a solutions‑focused approach
- Work collaboratively with families, support workers, clinicians, and other stakeholders
- Manage complex cases with an understanding of psychosocial disability, behavioural support, and crisis planning.
Requirements
- • Experience in Support Coordination, Case Management, Mental Health, Community Services, or Disability SupportRequired
- • Strong knowledge of the NDIS framework, funding categories, and compliance requirementsRequired
- • Excellent communication, time‑management, and organisational skillsRequired
- • Ability to work independently and manage a diverse caseloadRequired
- • Current Blue Card & NDIS Worker Screening CheckRequired
- • Valid driver’s licence and reliable vehicleRequired
- • High level of professionalism, confidentiality, and ethical practiceRequired
- • Experience supporting participants with psychosocial disability or complex needsPreferred
- • Qualifications in Social Work, Psychology, Community Services, or related fieldsPreferred
- • Understanding of SIL, SDA, behaviour support, and capacity‑building supportsPreferred
- • Familiarity with CRM systems and audit‑ready documentation processesPreferred
